Needle Gauge and Length for Syringes
A needle has two sizes: how thick it is (the gauge, written like 31G) and how long it is (in mm). Syringe needles from the makers we list come in 6, 8 and 12.7 mm lengths.

Gauge and length are two different things
Gauge tells you how thick the needle is (its outer width). Length tells you how far the needle sticks out. A higher gauge number usually means a thinner needle, but gauge tells you nothing about length. On a package, read “31G × 6 mm” as two separate facts: 31G is the gauge and 6 mm is the length.
Common insulin syringes in the US come with 30G × 12.7 mm, 31G × 8 mm or 31G × 6 mm needles. Other makers also sell 27G to 30G needles in some lengths. The shortest syringe needle in these listings is 6 mm. A 4 mm needle is a pen needle, not a syringe needle.
Needle sizes
How thick, how long?
A higher gauge (G) number means a thinner needle.
Length and thickness are drawn at the same scale, which is why the needles look so thin.
Needle sizes from: ISO 9626:2016, Table 1 — stainless steel needle tubing · embecta insulin syringe technical data · UltiMed insulin syringe specifications. Shown: syringe needles from 27G to 31G, in 6, 8 and 12.7 mm lengths.
What the gauge numbers mean in millimeters
Gauge sizes come from an international standard called ISO 9626. A 28G needle is about 0.36 mm wide, 29G about 0.33 mm, 30G about 0.3 mm, 31G about 0.26 mm, 32G about 0.23 mm and 33G about 0.21 mm. Each step up in gauge is only a few hundredths of a millimeter thinner.
These sizes don’t tell you which needle to use. Go by the gauge and length printed on your package, your product’s instructions, and what your healthcare professional told you.
Insulin syringes vs pen needles
Don’t lump the two together when comparing needles. The device sources we cite treat 4 mm/32G pen needles separately from insulin syringes. NovoFine Plus is one example of a 32G, 4 mm pen needle. The Ozempic instructions for use say you can use compatible needles up to 8 mm long, and to use a new needle for every injection. That’s about pen needles. It doesn’t turn a 4 mm pen needle into an insulin syringe needle.
For insulin syringes, embecta points to expert guidance that a 6 mm insulin syringe needle or a 4 mm pen needle works for all patients and injection sites. That’s a device company summarizing expert advice. It doesn’t mean you can choose a needle without checking your product and your healthcare provider’s instructions. The package has to match the device you’re using.
Use a new needle every time
The syringe maker embecta says to use a new syringe for every injection, because a needle is no longer sterile after one use. The Ozempic label also says to use a new needle for each injection and to store the pen without a needle on it. This has nothing to do with gauge or length: a needle that fits still can’t be reused.
When a vial’s instructions call for a syringe that can measure 0.5 or 0.6 mL, the syringe size matters as well as the needle. The Zepbound single-dose vial instructions don’t name a gauge or length. They give an example syringe and needle, and the line to draw to. Don’t guess a missing gauge from a different product’s instructions.
Common questions
What is the shortest insulin syringe needle in the listings you cite?
The shortest insulin syringe needle listed is 6 mm. 4 mm and 32G are pen needle sizes.
Does a higher gauge mean a longer needle?
No. Gauge and length are two separate measurements on the package.
Can I reuse a needle if it looks clean?
No. The device guidance we cite says to use a new syringe and needle for each injection.
